How to Apply for VA Disability for Back Conditions
To get VA disability for lower back pain or another service-connected back injury, there are several steps that you must take.

Gather Supporting Evidence
When applying for VA disability compensation, you must supply sufficient proof of the existence and severity of your injury, as well as proof that it was caused or worsened by your military service. To that end, the VA requires that you submit your DD214 or other separation documents, your service treatment records, and all medical evidence related to your condition. Lay evidence, such as witness statements, can be submitted using official forms available on the VA’s website.

Complete VA Form 21-526EZ
VA Form 21-526EZ is the standard application for disability compensation through the VA. You can:
- Print the form on the VA’s website and mail a completed copy to the Department of Veterans’ Affairs, Evidence Intake Center, P.O. Box 4444, Janesville, WI 53547-4444; OR
- Fill out and submit your application online through the VA’s eBenefits Portal; OR
- Submit your form in person at a VA regional office

Step
Attend a Compensation & Pension (C&P) Exam
This is a medical examination scheduled by the VA to assess the severity of your condition, and a key part of the VA claims process for back pain or other injuries. After you submit your application for benefits, the VA will reach out to you to schedule your C&P exam, which generally includes a physical examination and a thorough review of your medical records. This information will be used in support of either approving or denying your claim, and will play a significant role in determining your VA disability rating.
